Care Community has over 13 years experience in assisting & supporting Clients with spinal cord injuries, acquired brain injuries and multiple traumas following motor vehicle and work place injuries. Our disability support staff have an average tenure of six (6) years, meaning you can be assured of a regular and consistent team of health professionals who will work with you and your family on an ongoing basis.
We recognise the unique requirements needed to assist Clients and their families to make independent and informed life choices and so provide access to all relevant information and documentation in the development of your Independent Support Plan.
Our existing and long-standing Clients are testament to our ability to manage complex care needs in collaboration with families & the multidisciplinary support team.
Transition from Hospital/Rehab to home
Care Community’s relationship with you begins in the hospital/rehabilitation environment. Your personally appointed Care Manager will visit you & your family before your discharge home to participate in your Discharge Plan. This ensures a smooth transition from hospital to home.
Choosing your Care Team
Care Community encourages you to meet prospective members of your care team prior to commencement of services. We recognise how important it is to you to be absolutely comfortable with those involved in your Individual Support Plan. At any time you can ask to speak confidentially with your Care Manager if you wish to raise any issue about your care team.
Your Care Team
The Team appointed to your Individual Support Plan are appropriately qualified & experienced. They have specific training and expertise in such speciality areas as acquired brain injury, spinal cord injury, burns, multi system trauma, rehabilitation and Case Management. Your Care Manager is available 24 hours a day 7 days a week.
Regular Feedback
Care Community encourages all Clients and their families to meet on a regular basis to discuss progress of the Individual Support Plan.
