Q1) How do I access Care Community Services?

Q2) Do I need a doctors referral?
Answer
No, you or your family can self refer to Care Community. However if your needs are complex and involve such tasks as medication or clinical care we may need to contact your Doctor to obtain certain medical information. Any information received by Care Community about you is held in the strictest of confidence and is not released to anyone without your prior consent.
Q3) What services can I request?
Answer
No request is too small. Care Community offers a range of support services designed to meet your individual needs. This may be as simple as assistance with cooking and cleaning, transport to appointments, outings and shopping or even just a companion to walk with you or read to you. As your needs change, Care Community can provide more complex care such as assistance with personal hygiene through to specialised nursing care.
Q4) How much does it cost?
Answer
Care Community endeavours to keep its fees well within market range. The fees charged will be in accordance with the care you require and the type of care staff required to perform that care.

Q5) Can I call Care Community if I need to?
Answer
Yes, our Contact Centre is manned from 4.45am until 12 midnight 365 days/year. You may call at any time to speak with our consultants.
Q6) Can I discontinue the services, either for a short time or permanently?
Answer
Yes. Some services may only be required for a short period of time such as post hospitalisation, while other services may be ongoing. You may discontinue services at any time by giving Care Community 14 days notice in writing. We also request notice if you are intending to take a break from services. This might involve a holiday, a planned visit to hospital or respite care. This notice allows Care Community sufficient time to rearrange your schedule, find further work for your care staff and ensure that your regular care staff can be reassigned to you upon your return to the program.
Q7) How long can services be provided for?

For as long as you need them!
Q8) Can I have a say in who is assigned to provide my services?
Answer
Yes. You may discuss your individual preferences with our service consultants. Where possible we will endeavour to accommodate those preferences.
Q9) Once I Submit a Request for Care, What happens next?
Answer
One of our Nurse Consultants will contact you within 24hours of receipt of your request. They will make a time that is suitable with you to come to your home and discuss your request with you. You will need to read and sign an ‘Agreement’ that essentially contracts Care Community to provide your care services. You will also be required to sign a financial agreement that covers the agreed fees and payment method. Once all of your information has been collected, we can usually commence services within 24 hours.
